Like others, I'd go with "First Gentleman" as the most likely counterpart to "First Lady"

No. In English, the complement of "Lady" is "Lord", so he would be called the First Lord.

In Australia, we have the first female prime minister and we don't have much of a name for her partner. It'll be interesting to see when there is a female president on board in America.

The male counterpart of “The First Lady” should be “The First Gentleman.”
The male counterpart of “The First Woman” should be ”The First Man.”

Therefore, the husband of a female President should be called “The First Gentleman” in English.

Is it “El Primero Caballero” in Spanish ? Or, is it “El Primero Señor”?

What was Néstor Kirchner called when his his wife, Cristina Fernández, succeeded him?
Succeeded him as the President of Argentina.

The Spanish shows what is correct. The complement to "Senora" is "Senor". El Senor is "the Lord".
