From what I read, the fire was an electrical fire, so it was most likely small and contained. In a sealed plane cabin, his wife probably suffered a MUCH greater chance of harm from the smoke than from the fire itself.
A) Open a giant hole in the fuselage big enough to cause explosive decompression
B) Choke to death
Back in the 60s and 70s, they used to filter more outside air into the pressurized cabin. But with each passing decade, they look for better fuel economy so they shoot for less drag...all the while sealing the cabin tighter. By the end of the 90s there was a 30% drop in outside air ventilation. Apparently it's a big deal with all the health problems that stem from the stale air.
