Most of these decisions are driven (either directly or indirectly) by idiotic US tax policy. My version of tax reform would cut useless overhead by firing every corporate tax accountant in the country. With a newfound emphasis on dividends, even CEO compensation would get hit when the board of directors starts squeezing the entire enterprise for dividends instead of tax dodges.
Or we can just clamor for "Carter II" to let the inflation genie out of the bottle. Because that worked so well in the mid 1970's? Really?
There are many RAGS TO RICHES stories out there in this world. Not their fault some people have no drive and just want to settle for whatever is easy. Also those minimum and median workers should thank those CEOs for keeping businesses successful, hence providing them jobs.

Minimum Wage Earner - Unskilled and minimum education, needs to be trained and is responsible only for the work he does.

CEO Guy - Highly skilled and educated and years of experience and is responsible for thousands of peoples work, the entire companies production, sales, safety, product quality, product development, marketing, regulation compliance, inventory control, shipping, purchasing, workers health insurance, retirement programs, staying competitive, pricing, and held accountable by the stock holders of multimillion dollar corporations.
If the "99%" don't like their position in life, don't hate others and try to bring them down, but try to improve and raise yourself. Envy and jealousy will only bring you misery!

Whether milk costs a dollar a gallon or ten dollars a gallon the point is that it takes a disproportionate amount of work and effort for the average person to buy it than it does for the CEO. It is that gross disproportionality that is the problem. Thirty years ago the impact would have been about 20 to 1. Now it's 2000 to 1. Over the past five years ninety five percent of the wealth generated by the recovery has gone to the top 1% while the earning level of the average American has been flat or has even gone down (adjusted for inflation).
