Usually only specific people get it. Elderly in nursing homes, bed ridden people, etc... but, apparently can include: "some otherwise healthy people who aren't hospitalized or taking antibiotics"
That was me. They kept asking me, "Have you been around a nursing home?" or "Have you been taking antibiotics?". They couldn't believe someone my age (30's at the time) could get it without some sort of external genesis. Worst pain I have ever felt. They injected some "oh my God that stuff works great" pain medication in my IV every hour. They asked for a stool sample after a few hours when they still didn't know what it was. I said "See that large pool of blood at the bottom of your sample thing you asked me to use an hour ago? That IS the stool sample". She simply thought I hadn't gone yet so kept coming back to check, as it probably seemed empty on just a glance.
The nurse's eyes got big and people got antsy after that. They figured it out after about 4 hours of being there, and were explaining this to me as they quickly and quietly moved me to my own room. C. Diff infection spreads rather easily, apparently, and I had one of the worst cases some of these nurses had ever seen. The nurse actually scared my best friend fro...

A long, gruesome story later, still no idea. Pictures of the infected area didn't look like much to me, but the doctor was quite impressed with the magnitude. Asked me more questions that I could say "no" to. "No, I didn't help my grandma move something at her house" (not kidding), etc... . I am an active guy, pain is just something I allow to happen (I do not complain about pain a lot). Two days earlier I was totally fine, the debilitating pain forcing my wife to rush me to the hospital came on after only about 2 hours after first "Hmmm? Not sure if that feels right...". Fast.
Meh. There is my completely unnecessary, far too long, utterly gross, story from a bored socokid. Now the Sox game is on. Have a great night!

Well, mine was like that only double that.
I'd say ten times worse but that'd be exaggerating.
