It's completely a formality. I'm resentful toward anyone who says much more than, "fine, thanks" -- especially if I'm working in the service industry and ask you. Big hint -- if someone's getting paid to ask you how you're doing, they really don't give a $hit.

Somewhere in the middle. Many people use "How are you?" as a generic greeting and are not in least bit interested in your well being. I don't ask if I'm not interested. But, do keep it reasonable short. If I want more detail, I'll follow it up with additional question(s.)

It depends on who I am asking. If it's just an acquaintance, you know, you run into them and say "Oh hey, how are you doing?" then a summary is just fine. If it's more of a close friend, or boyfriend, or family member, I am happy to hear it in a lot of detail :)

I intensely dislike people who use "how are you" as a greeting because they tend to not even wait for a reply at all. That's quite rude imo.
