I fear:
Death because I don't know where I would go if died.

I used to have a few, but I did an Outward Bound Program years ago that taught me the difference between fear and anxiety, and reset my internal gauges so what was once a 10 turned out to be a 2 or 3. After rappelling down 100' lines into open gorges, I realized that most things in life won't kill you let alone hurt you, so I don't sweat the small stuff.
