Only if you are family by blood and I see you as a family to me. Fortunately, there has not been a point where someone messed up really bad and I had to give them a second chance for being there for the individual.

it so much easier to give someone a second chance than to continue to be mad at something. Just let it go, learn from what happened, and move on. It takes to much work not to forgive
Involving most things. Forget my birthday, sure, have a few chances. Lie about something big(like a drug problem), I'll give one other chance. Cheat, maybe but probably not. Murder someone close to me, bye bye, hope the afterlife looks nice.

But honestly? In a way, no, I don't. If someone breaks my trust, that's the end of that. Doesn't mean I'll do anything to push them out of my life, but they aren't fixing that trust.
