the scariest movies are those which allow your imagination to run free - blood and gore just gets tedious after a while and in regard to the screams, painful to the ears.

I just watched the Human Centipede a couple of weeks ago. I seriously am disturbed by that movie. I think a couple of times I tried to throw up. It really is disgusting.
I'm never really all that scared...I used to be terrified, but then I had some real life scary things happen to me and my family, sooo now I'm not scared. I rarely have fear about movies.
Inland Empire, Literally gave me nightmares for a week, yet I've watched it 4 times again since then. The movie is directed by David Lynch, it's over 3 hours long and just seems like an endless nightmare at times.
However it has something artistically beautiful scenes too and impressive dialog.
For scary I would say the grudge, the exorcist, the ring, or Carrie maybe...
For holy crap I'm gonna barf disturbing kinda horror, Human Centipede or Antichrist.
Bob fainted during both of them. He went down at the part where the girl rips the artery in her arm in the human centipede and the log/scissors scene in Antichrist.
It's a toss up between two movies both of which I viewed when I Was 5 years old.The Crawling Hand,when that hand sneaked under that woman's pillow and then strangled her,well I checked underneath my pillow for years after.The corpse in the rocking chair trying to get back the ring that floozie took off her body-UGH! still gives me the heebee jeebees
Knowing that film is film and nothing is true there, the last scene can make some sleepless nights ...
