How about addressing a woman by their name. If they don't know her name then Miss or ma'am works as well as saying what needs to be said minus a title. How hard is that?

I don't like being called 'girl' and haven't since I turned 18. I usually respond in kind. If it is someone I will have to have continued contact with, I tell them that it is insulting and please not to do it again. They usually don't after that.
