I was in a porn for public disgrace . com and the way they did was they rented our a coffee shop and hired people to be extras. That is what I was an extra.

They rent a place (bar, subways, public area), and fill it with volunteers, that know what's going on and what's going to happen. If someone is against it, then they aren't allowed in the area. The volunteers might get paid nothing, or might be paid a very very little amount.
They're are some true public ones, but they're usually try to keep it low key with no one seeing... if a lot of people are seeing or it's out in the open, then they probably rented the place or area for a video shoot.
