How are you? Do you answer honestly?

I've always kind of found the question "How are you" to be a rhetorical question. I answer "Fine, and you?" and I realize, I they probably really don't give a hoot on how I really am feeling.
Ever since I let the cat out of the bag that I'm pregnant, I am constantly asked "How do you feel?" My answer is always "Oh, pregnant," or "Depends on the day." When in all actuality I'd love to say "I'm crazy nauseous, my head hurts, I'm uncomfortable, stressed to the max..." etc. But I know, they really don't give a hoot.
So my question is, should we really answer honestly when someone asks "How are you?" Or, what would your answer be if you were honest when asked the question?

Next, Im usually honest, when asked "How are you?":
"I've been better, I've been worse."
That's an honest answer, rather than a mere placation.
