A couple of times. I don't see it as a problem, though. I consider my opinion and passion to be right or I wouldn't post it. Some people just don't want to respond to it or me to respond to them. No biggee...

Usually only in political discussions though. Some Liberal will get offended by pro gun statements, or some Conservative will block me for saying that equal rights/privileges are for the best (whether they be sexual, race, gender, class, etc.). Or blocked by both while arguing about the evils of the income tax in general.
Of course. I get called names, and then I get blocked. I haven't ever blocked anyone; I just ignore the ones who can do nothing much more than call me names, or drive home their "point" by name calling. I think the least offensive name, lately, has been "maggot". Do I care? Not so much.....
