Does the term 'knocked up' mean unmarried and pregnant or just pregnant?

So I'm having an argument with my boyfriend, He says that any pregnant woman is 'knocked up' which I reply with that the terminology may only apply to a woman who is unmarried because a woman who is married isn't knocked up it's just assumed that it's 'part of the plan'....do you think the phrase "Knocked up" means a general phrase or just for an unplanned unmarried pregnancy?

On a similar note, I recently learned that saying "I'm stuffed" means that you are pregnant, in Australian slang.
