There are four main causes of cerebral (ischemic) infarctions:
1. Large vessel disease - infarction occurs as a result of embolism or thrombosis of a large (named) cerebral artery; the resulting infarct involves the territory of the brain supplied by that artery
2. Small vessel disease - usually caused by arterisclerosis of small penetrating vessels within the basal ganglia or pons
a. often associated with hypertension or diabetes mellitus
b. resulting small infarcts are called "lacunar infarcts"
3. Global ischemia - caused by a reduction in cerebral blood flow (as in cardiopulmonary arrest from various causes), which results in widespread neuronal necrosis and possible laminar cortical necrosis
4. Venous infarction - least commonly seen
a. seen with thrombosis of the main cerebral venous sinuses, which leads to hemorrhagic necrosis
b. may occur in conditions that predispose to thrombosis, such as polycythemia, dehydration, or spread of infection from nearby structure such as the middle ear or nasal sinus.
